Include all WINZ Forms, Birth Certificates, Medical Information & Special Dietary Requirements
If you have a current enrolment form you may be able to simply fill a re-enrolment form. Contact us for advice.

Download forms or visit our office and we will provide them

Your'e done!!
Email, Post or Drop-in all the completed forms to us.
Once we've received your application we will check through it and get back to you within 3 - 5 business days
